COVID-19 Disability Form

The COVID-19 Disability Form was created to help friends, family members and clients receive quality medical treatment.  The form was reviewed by physicians, and families of individuals with intellectual and developmental disabilities.  Having the form, and the accompanying documents noted on page 2 of the form, will help alert the hospital staff.  So, the form will give the preferred communication means of the individual.  Also the needs and wishes of the individual with intellectual and developmental disabilities seeking medical treatment are detailed.

The goal is to complete the form proactively.  Have the form completed prior to seeking medical attention for COVID-19 symptoms.  Dr. Ballan developed the COVID-19 disability form as to meet the quickly changing needs in the Metro NYC area.  Because NYC had new guidelines, caregivers were not being allowed to enter the hospital setting with the individual with intellectual and developmental disability.  The form was then amended by each state governmental agency’s developmental disabilities services.  You can access the website at https://you.stonybrook.edu/…/preparing-individuals-with-in…/
