Taking Charge of My Health Care

Georgia Department of Health publishes “Taking Charge of My Health Care- Moving from Pediatric to Adult Health Care- A Workbook for Youth and Young Adults With or Without Disabilities”. This workbook is designed to be a useful tool for planning and practicing for the transition from pediatrics to adulthood.  One is encouraged to review the workbook, and take the information that works for themselves and their family, and make it their own. You are encouraged to have fun , ask questions and write things down that your provider shares with you about your health care.  Most importantly, remember that you have a team supporting you on this next step to living independently. The workbook is for ages 13 to 21 years. Topics including understanding your health care, communicating and evaluating choices.  In addition, there is a section that discusses “Paying for Health Care” in general terms and options.  Resources are also given.
